We are so excited to welcome your family to the Hazel Fischer Community!

To get your child registered at Hazel Fischer Elementary School, we only need a few documents and completed forms (see below).

Along with the completed Student Registration Packet below, please also provide the following three documents when registering your child:

• Birth Certificate
• Immunization Record
• Three proofs of physical address as explained in the Proof of Residency Requirements below
